2009 State Benefits Open Enrollment
IMPORTANT UPDATE
All benefits-eligible faculty and staff must take action - Elect or waive all plans AND provide all dependent social security numbers (SSNs) - no later than 5 p.m., Sept. 4, 2009, to continue benefits after Sept. 30, 2009. Coverage will take effect Oct. 1, 2009.
Follow the above link to enroll now!
If you fail to take action:
- You will be randomly enrolled in one of the medical EPO plans, effective Oct. 1, 2009.
- Your dental, vision, life and disability benefits coverage will expire Sept. 30, 2009.
- Your dependents’ benefits coverage will be cancelled if you do not provide their SSNs.
